In a recently published randomised trial of 886 subjects who were willing to quit smoking [100], the abstinence rate was found to be twice as high in the e-cigarette group than in the nicotine-replacement group (18.0% vs. 9.9%) after 1 year.
Silica was commonly the first material to be used as a wick, followed by cotton and ceramic3,13,14,15. Cotton has good wicking properties but is less thermally stable than silica14,16,17, while ceramic is chemically stable and heat-resistant18. The use of microporous ceramic as a wicking material has increased in the past few years14,16,18,19,20. Its application has been reported to improve heating efficiency and reduce charring14,16,18,19,20. Additional research is ongoing to clarify the dangers from vaping, and how vaping damages the lungs and other organs. Long-term human studies, in particular, could bring the risks from e-cigarettes into better focus, Crotty Alexander says.

Founded in 1913 as the Harvard-MIT School of Health Officers, the School is recognized as America’s oldest professional training program in public health. This study explored the prevalence of S-SM, S-EC, and S-SN among adolescents in a country with advanced tobacco control regulations. We found that the susceptibility was highest for e-cigarette use, followed by smoking and snus use. S-SM was more common among girls, whereas S-EC and S-SN were more common among boys. In our analysis, levels of carbonyls were considerably reduced relative both to other studies of e-cigarettes and to Ky1R6F cigarette smoke.
In Cumberland County, Chalmers McClain and Graham said the growing number of vape shops opening near schools with their colorful and bright lights to attract students is also part of the problem. For example, Ramsey Street Alternative High School has two vape shops within a six-minute walk from the school. Teacher concerns around increasing the suspension rates of students caught vaping on school property prompted the initial pilot and current implementation of CATCH My Breath into county schools, said Graham.
It would seem that future studies should analyse the possible toxic effects of humectants and related products at concentrations similar to those that e-cigarette vapers are exposed to reach conclusive results. In this line, a study compared the acute impact of CS vs. e-cigarette vaping with equivalent nicotine content in healthy smokers and non-smokers. Both increased markers of oxidative stress and decreased NO bioavailability, flow-mediated dilation, and vitamin E levels showing no significant differences between tobacco and e-cigarette exposure (reviewed in [20]). Inasmuch, short-term e-cigarette use in healthy smokers resulted in marked impairment of endothelial function and an increase in arterial stiffness (reviewed in [20]). Similar effects on endothelial dysfunction and arterial stiffness were found in animals when they were exposed to e-cigarette vapor either for several days or chronically (reviewed in [20]).
Research from The Johns Hopkins University on vape ingredients published in October 2021 reveals thousands of chemical ingredients in vape products, most of which are not yet identified. Among those the team could identify were several potentially harmful substances, including caffeine, three chemicals never previously found in e-cigarettes, a pesticide and two flavorings linked with possible toxic effects and respiratory irritation. "These cases appear to predominantly affect people who modify their vaping devices or use black market modified e-liquids. This is especially true for vaping products containing THC," explains Blaha. However, a major concern about vaping is its attraction for young people. And, while it’s safe when taken orally as a supplement or used on the skin, it’s likely an irritant when inhaled. It’s been found in the lungs of people with severe, vaping-related damage. Some of these additives have health risks, such as diacetyl, which has a buttery taste. In December 2019, Congress raised the minimum age to buy tobacco products, including e-cigarettes, from 18 to 21. Because of the popularity of certain flavored e-cigarette products among children, FDA stated in January 2020 that it would prioritize enforcement actions against the manufacture and sale of most flavors in cartridge-based e-cigarettes. In April 2020, FDA extended from May to September 2020 the deadline for manufacturers to apply for premarket authorization, a process that includes the scientific evaluation of risks and benefits of e-cigarettes for the U.S. population. In 2014, just under 8% of Wisconsin high school students were using e-cigarettes. In 2018, that number has skyrocketed to 20% (or one out of every five students[i]). They found that while several of the liquids were moderately toxic to the endothelial cells, the cinnamon- and menthol-flavored e-liquids significantly decreased the viability of the cells in culture even in the absence of nicotine. The tobacco cigarette used as a comparator was the Kentucky Reference Cigarette 1R6F (Centre for Tobacco Reference Products, University of Kentucky, USA), which has been designed to provide a standard test piece for scientific studies. It is an unflavoured US-blended king-sized product with a cellulose acetate filter, an aerosol nicotine level of 1.9 ± 0.1 mg cig–1, and a tar yield of 29 ± 2 mg cig−1 as measured by the ISO Intense smoking regime6,56.

One study detected vitamin E acetate in the lung fluid of 48 out of 51 EVALI patients sampled across 16 states. By contrast, lung fluid samples taken from healthy people did not contain the vitamin. EVALI is a serious medical condition in which a person’s lungs become damaged from substances contained in e-cigarettes and vaping products. The smoke-free and tobacco-free policies at schools, businesses, healthcare institutions, and other organizations should also cover e-cigarettes. This will help non-users avoid being exposed to potentially harmful e-cigarette aerosol.
Upon the selection of the educational track, students may already have normative perceptions of smoking in vocational schools, such as understanding of smoking as an integral and expected practice in these schools and professions [8]. These perceptions may increase S-SM and other nicotine product use, also among students with better academic performance. Student network characteristics and their associations with smoking vary between schools [28].

Other appeals courts have sided with the FDA, which regulates new tobacco products under a 2009 law aimed at curbing youth tobacco use.
According to Blaha, there are three reasons e-cigarettes may be particularly enticing to young people. Second, e-cigarettes have a lower per-use cost than traditional cigarettes. State and local authorities can restrict the sale of flavored tobacco products, including e-cigarettes.
The cells are also less able to form new vascular tubes and to migrate and participate in wound healing. With a 399.73% increase in retail e-cigarette sales (excluding internet sales and tobacco-specialty stores) from 2015 through 2020, the environmental consequences of e-cigarette waste are enormous. The fourth-generation vaporizers can also be customized and come with different types of heating coils — some intended for vaporizing solids, not liquids. In general, people using e‑cigarettes did not report doing so in order to quit smoking regular tobacco cigarettes. In 2022–2023, only 1 in 5 (21%) people who had ever used e‑cigarettes reported that they first used e‑cigarettes to help them quit smoking (Figure 5). The most common reason people gave for using e‑cigarettes was out of curiosity (58%).
However, the long-term effects of all flavour chemicals used by this industry (which are more than 15,000) remain unknown and they are not usually included in the product label [78]. Furthermore, there is no safety guarantee since they may harbour potential toxic or irritating properties [5]. In addition to its toxicological effects on foetus development, nicotine can disrupt brain development in adolescents and young adults [44,45,46]. Several studies have also suggested that nicotine is potentially carcinogenic (reviewed in [41]), but more work is needed to prove its carcinogenicity independently of the combustion products of tobacco [47].

While there appears to be some consensus that vaping may be less harmful to health than tobacco cigarette smoking, the dangers of e-cigarettes remain unclear. The range of e-liquid flavours available to consumers is extensive and is used to attract both current smokers and new e-cigarette users, which is a growing public health concern [6]. In fact, over 5 million middle- and high-school students were current users of e-cigarettes in 2019 [75], and appealing flavours have been identified as the primary reason for e-cigarette consumption in 81% of young users [76]. Since 2016, the FDA regulates the flavours used in the e-cigarette market and has recently published an enforcement policy on unauthorised flavours, including fruit and mint flavours, which are more appealing to young users [77].
Differences between schools exist, but having more students with no nicotine use showed protective associations, while having more students with positive attitudes towards product use in one’s age group increased the susceptibility. The results of the present study suggest that both interventions tailored by educational aspirations and universal school policies are needed to reach youth at high-risk of initiation. The concentration of nicotine in the BB57 emissions relative to BB18 was three times greater and followed the three-fold increase in nicotine strength of the e-liquid. In the cigarette smoke, the measured nicotine concentration of 0.23 ± 0.01 mg puff–1 (Table 1), equivalent to 2.07 ± 0.09 mg cig−1, is in accordance with the Ky1R6F certificate value of 1.9 ± 0.1 mg cig−1 (ISO Intense smoking regime)56.

They observed, on average, a 34 percent reduction in the femoral artery’s dilation. These findings suggest that vaping can cause significant changes to the inner lining of blood vessels, said study lead author Alessandra Caporale, PhD, a post-doctoral researcher in the Laboratory for Structural, Physiologic, and Functional Imaging at Penn. In this study, the researchers examined the impact of an e-cigarette that contained propylene glycol and glycerol with tobacco flavoring, but no nicotine, which study participants took 16, three-second puffs from. To evaluate vascular reactivity, researchers constricted the vessels of the study participants’ thigh with a cuff and then measured how quickly the blood flowed after its release. Using a multi-parametric MRI procedure, researchers scanned the femoral artery and vein in the leg before and after each vaping episode to see how vascular function changed.
Besides nicotine, e-cigarettes and e-cigarette vapor typically contain propylene glycol and/or vegetable glycerin. Researchers spent decades studying the health consequences of secondhand smoke. It might be a while before we have a clear picture of how secondhand vaping can impact a person’s long-term health, especially for young children.
In our study, there were between-school differences, which appeared larger for girls than boys. In the univariate analyses, most school-level factors either increased or lowered susceptibility. This was especially the case among girls in S-EC and S-SN, and among boys in S-SM. In the multivariate models, most of the school-level associations were weakened.
